About the Culinary Extra Clinic and Iora Health
The Culinary Extra Clinic is a primary care practice in Las Vegas, NV dedicated to serving the needs of participants in the Culinary Health Fund who have one or more chronic illnesses.  The Clinic uses an innovative care model in which physicians and health coaches partner with patients to help improve overall health.  We do this by building trusting relationships, providing a single place for each patient’s primary care needs and collaborating with patients to proactively address unmet health needs and concerns.  Iora Health is a Cambridge, MA-based company that manages the Culinary Extra Clinic in a close partnership with the Culinary Health Fund.

Job Description
Reporting to the Nurse Innovator, the Health Coach serves as a key member of the Iora Health team and plays a critical role in helping patients improve their health.  Health Coaches engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt life-long healthy behaviors. Our Health Coaches help patients unlock personal motivation to make lasting changes and simultaneously serve as the patients’ primary conduit to the practice as they help coordinate necessary services both within and outside of the practice.

In addition to their active clinical involvement, Iora Health Coaches rotate through the roles of greeting patients to the practice and providing patient support wherever needed, allowing them to influence the patient experience in its entirety, from the moment patients set foot in the practice to the time they leave and well beyond. We believe understanding and fostering the continuity of the relationship helps build the strength of the collaborative culture Iora Health embraces.

Specific responsibilities include, but are not limited to the following:

• Collaborating with colleagues to actively engage 200+ chronically ill patients in their care
• Communicating with patients in person, in groups, by telephone and via electronic means
• Implementing a range of interventions/skills including counseling, motivational interviewing, coaching,, education, referral and advocacy to gain access to services and resources for patients
• Providing customized, evidence-based patient education in a variety of areas including but not limited to weight management, tobacco cessation, and chronic disease self-management
• Communicating care plans and pertinent information with the internal Culinary Extra  team as well as with other parties participating in the care of the patient
• Assisting with the coordination of care across the care continuum (including home care, outpatient care, ER care, and hospital care) while maintaining strict patient confidentiality at all times

Candidate Attributes

• Experience in a service environment required - can, but must not be in a health care setting 
• LPN or MA preferred, but not required
• EMT or Certified Athletic Trainer a plus
• Spanish language fluency required
• Must serve as a role model and be able to “walk the wellness walk,” not simply “talk the wellness talk”
• Must be a member of, and familiar with, the Las Vegas community
• Must be open-minded, non-judgmental and compassionate
• Must demonstrate excellent interpersonal communication skills including exceptional listening skills
• Must have the ability to work well both as a member of a team and independently
• Must have exceptional organizational, problem-solving and time management skills
• Must demonstrate intellectual independence and willingness to learn new skills
